Children in the foster care system are regularly involved with more than one human service agency or system which can lead to lack of appropriate service coordination between entities and ultimately result in restrictive out-of-home placements. Wraparound seeks to holistically support children involved in mental health and child welfare systems in their communities through person-centered and family driven approaches, including the development of a customized care plan which outlines the child’s needs, natural supports, and preferences. In this webinar hosted by the National Center on Advancing Person-Centered Practices and Systems, panelists will provide an overview of wraparound, discuss the importance of partnership and collaboration between agencies to support children, and highlight person-centered approaches to developing the plan of care.
